• Deposit Agreement Form  
• Designated Agent Form (if person coming in is not the company owner)
• Copy of your NM Contractors License W-9
• Valid Driver’s License
• Required payment of $3241.50 ($3000.00 A Loss, Damage and Payment Surety Deposit plus a nonrefundable $241.50 connection fee)
