To get started, just complete the form below and attach a voided check from your bank checking account or a deposit slip from your savings account. Mail it to the address noted below. Automatic payments will begin on the first billing statement following receipt and entry of automatic payment information (approximately 45 days).

You can stop the automatic payment plan at anytime by calling 768-2800. What if I change financial institutions?  Just call us at the number above. It will be necessary for you to fill out and submit a new auto pay form with the updated information along with a voided check from the new banking institution.
